NOTES: - This is a reprint copy of a 50+ year-old original document. Some imperfections may be present in the reprint due to existing condition of the antiquated original, including some light/faded printing that may be hard or impossible to read, some details that may be obscured due to folds/creases/tears in the original document, etc. If you have any specific questions regarding condition, please ask before ordering, as paperwork items are not returnable.
