Replaces original opto board #s A-14977, A-15576, A-15576.1, A-15595, A-15595-1, and bare unpopulated PCB #s 5768-12921-00, 5768-12921-01, 5768-12921-02, 5768-12921-03, 5768-12921-04, and 5768-12921-05.
